Please provide a written bid on the following services for SSA #31

Terms: April 1, 2010 to November 1, 2010 with option to extend contract on weekly basis.

The following services to be provided three times a week:

• Remove trash from the sidewalk/curb/gutters including 10 feet in the alleys
• Pick-up and remove litter and trash from front sidewalk/ curb/gutter areas including mouth of alleys and 10 feet in to alleys.
• Empty all public trashcans along the route.

• Billing to occur on a monthly basis.

TERMS:
Please refer to Chris Shickles for approval for all schedule adjustments.

SSA 31 will provide orange vest with the name of your organization and “SSA # 31” on the back to be worn while working as well as magnetic “SSA # 31” signs for vehicles.

Each contractor shall provide its services and meet its obligations under this Contract in a timely and workmanlike manner. All payments are contingent on SSA #31 Commission approval accompanied by typed invoice and may require a maximum of 60 days before completion of payment. Each contractor must provide proof of insurance as well as listing Ravenswood Community Council and SSA #31 as co-insured. Ravenswood Community Council will not be held responsible for damage to equipment or personal property.
