Please provide a written bid on the following services for SSA #31 Clark/Lawrence Ave.

Location: The parkway between Lawrence Ave. and Ainsle along the Metra line.

Services needed:

March 2009: Spray all quackgrass areas with the herbicide Ornamec

LUMP SUM: $______________

Mulch entire parkway with 4” wood chips to be provided by the City

LUMP SUM: $_______________

May 2009: Weed entire parkway, removing all herbaceous material that has come up through wood chips, with particular attention to Garlic Mustard.

August 2009: Cut entire parkway to ground level; bag for City removal

LUMP SUM: $______________

Remove all vines from trees and shrubs; prune any dead limbs

LUMP SUM: $______________

Each contractor shall provide its services and meet its obligations under this Contract in a timely and workmanlike manner, using knowledge and recommendations for performing the services which meet generally acceptable standards. All final dollar amounts must not exceed the specified amounts indicated at time of bidding. All payments are contingent on SSA #31 Commission approval accompanied by typed invoice and may require a maximum of 60 days before completion of payment. Each contractor must provide proof of insurance as well as listing Ravenswood Community Council and SSA #31 as co-insured. Ravenswood Community Council will not be held responsible for damage to equipment or personal property.
